# Agency and team review worksheet

This blank worksheet organizes a review. It is not a validated scoring model, an employment assessment, a performance forecast or an automatic decision to change a supplier.

Use one row per question and one consistent review period. Compare evidence covering the same project and scope. If the evidence is unavailable, record that status instead of treating an assumption as a verified finding.

| Field | Meaning |
|---|---|
| review_period | Start and end dates covered by the evidence |
| work_area | The responsibility being reviewed, such as media buying, measurement, creative, sales handoff or costs |
| review_question | The concrete question the evidence should resolve |
| evidence_reference | A non-sensitive document label or approved source reference; not credentials or a private access URL |
| observation | What was actually observed, including scope and limitations |
| status | For example: verified issue, no issue found in scope, hypothesis, missing evidence, action agreed or checked after action |
| responsible_role | The role responsible for the next action; no personal contact details are needed |
| agreed_action | The action the relevant owner has actually agreed to; a proposal is not an authorization |
| acceptance_check | The evidence that will show whether the action was completed correctly |
| next_review_date | The agreed review date, not a promised outcome deadline |

Do not put customer names, phone numbers, messages, payment details, access tokens or passwords in this worksheet. Keep detailed source data in its approved system. Before sharing, remove private links and check access to any referenced material.

For a cost review, state which period, work and costs are included; distinguish media spend from service fees. Unknown quotes remain unknown. For a lead review, reconcile the same cohort and follow-up interval. A blank finding or an unavailable denominator is not a zero cost or a passing grade.
